Who Can Receive Care?
Anyone who lives in the Polk County area and is unable to receive medical or dental care due to lack of insurance may be eligible, based on the following requirements:

Eligibility Requirements:

1. You must live in the Polk County area for at least three (3) months.
2. You do not have medical insurance.
3. Your income does not exceed 200% of the Federal Poverty Level.
