WHO CAN RECEIVE CARE?

Anyone who lives or works in the greater Lakeland area and is unable to receive medical or dental care due to lack of insurance may be eligible, based on the following requirements:

Eligibility Requirements:
1. You must live or work in the greater Lakeland area for at least three (3) months.
2. You do not have medical insurance.
3. Your income does not exceed 150% of the Federal Poverty Level.
