We’re Hiring: Healthy Living Coach

 

Lakeland Volunteers in Medicine is seeking a compassionate and motivated Healthy Living Coach to join our team.

This full-time position will work closely with patients diagnosed with diabetes or pre-diabetes, helping them make healthy lifestyle changes, achieve personal wellness goals, and improve their overall health. The Healthy Living Coach will provide one-on-one support, develop health plans, lead diabetes education programs, and collaborate with LVIM providers to help patients succeed.

We are looking for a Registered Nurse who is passionate about patient education, preventive care, and making a difference in the lives of others.

At LVIM, you’ll have the opportunity to help uninsured and medically underserved residents of Polk County live healthier lives while working alongside a caring and dedicated team.
