NOTICE:  The LVIM Pharmacy will be closed on Friday, September 25th while we update our pharmacy software. During this time, no medications will be filled or signed out. The pharmacy will reopen on Monday, September 28th. Thank you for your patience and understanding during this transition.

LVIM is growing!

Thanks to incredible community support, LVIM will be moving to a new facility in the spring of 2019.  This new facility will provide 50% more clinical space to help meet the growing need to serve Polk County’s working uninsured population.  We hope you’ll check back here for news and updates as our story unfolds.
